    Complaints Policy Sapphire Dental Code of Practice for Patients Who Wish to Raise Concerns ​ In this practice we place great emphasis on meeting and whenever possible, exceeding our patients expectations. We try to ensure that all patients are pleased with their experience of our service and we take any concerns a patient may have very seriously. If you have a concern regarding any aspect of your care, please let us know. We will do all that we can to resolve your concern to your satisfaction both promptly and professionally. ​ Private complaints can only be addressed when raised within 12 months of the treatment taking place or within 12 months of becoming aware that you have something to complain about. ​ Our aim is to respond to patients concerns in a caring and sensitive way. ​ The persons responsible for dealing with any concerns about the service we provide in this practice are the Principal Dentists, Dr Manoj Tank and Dr Nisha Sanghrajka. ​ If a patient raises an issue they are unhappy about on the telephone or at the reception desk, after listening to a description of the problem, we will immediately contact the Principal Dentist. Should the designated person or other responsible person not be available at the time, the patient will be informed when they will be available and arrangements will be made for the two to meet or speak. The member of staff will take brief details of the concern and pass them to the Principal Dentist. ​ If a patient writes to express their concern, the letter will be passed on to the relevant person immediately. ​ We will acknowledge any concerns raised in writing within two days. If a concern has been raised in writing, the written acknowledgement will also include a copy of this Code of Practice, again, normally within two working days. We will investigate the concern and report back within ten working days of it being received. If we are unable to complete our investigations within ten working days for any reason, we will notify the patient, giving reasons for the delay and the likely period within which the investigation will be completed. ​ We will confirm the outcome of the investigation and any decisions made in writing. Proper and comprehensive records are kept of any concerns or complaints received.     Facial Aesthetics Book a Consultation Facial aesthetics are non-surgical treatments to soften fine lines and define features Nisha Sanghrajka offers wrinkle reduction and lip filler treatments at Sapphire Dental. Wrinkle reduction ​ Nisha uses Azzalure, a muscle relaxant derived from Botulinum toxin type A, to smooth out fine lines and wrinkles. It is commonly used to smooth out forehead lines, crow’s feet and frown lines. ​ Azzalure is administered using a very fine needle to target fine lines and wrinkles and ensure a more youthful appearance. It is a quick procedure, making it highly convenient to fit in around your schedule without taking any time off to recover. You may notice a little redness or swelling following the procedure, but this is normal and should disappe ar within a few hours. ​ Results are very natural-looking and can last around 3–5 months; regular top-up treatments ensure lasting results. ​ Lip fillers Nisha uses Teoxane lip fillers to provide a hydrated, more defined appearance of the lips. ​ Teoxane is an injectable hyaluronic acid-based dermal filler designed to improve upper and lower lip volume as well as the philtrum (the lines between the nose and the upper lip). ​ Teoxane is injected into the dermis to immediately alleviate signs of ageing. Its high concentration of hyaluronic acid gives volume to the face and lips. For your total comfort, this treatment is carried out under local anae sth etic. ​ Some redness, swelling, bruising or tenderness may occur at the injection site, but this is usually transient.     Children's Appointments Many of our patients ask when their child can have their first check-up at the dentist. The answer is there is in fact no age restriction on taking your child to the dentist. Mostly, when a child's first tooth comes through the gum, usually at the age of 6 months, it's a great time to book their first trip to the dentist. At Sapphire Dental we also have our children's corner where many of our younger patients enjoy colouring, building tower blocks and also knocking on our Tooth Fairy's door! It is important to maintain children's oral hygiene in order to prevent tooth decay, which is the primary reason that children have a general anaesthetic. We recommend parents to help children brush their teeth until at least the age of 7, and certainly until they have developed the dexterity to manoeuvre a toothbrush around their mouth effectively. Children's appointments are never rushed and we work with the parent to create a friendly environment so they can enjoy their visit to the dentist and leave with lots of positive experiences.     Privacy Policy ​ Privacy Policy for Sapphire Dental for Patient Data ​ Sapphire Dental takes great care to protect the personal data we hold for our patients in line with the requirements of the General Data Protection Regulation (GDPR) and the Data Protection Act 2018. ​ The purpose of collecting and storing personal data about our patients is to ensure we can: Provide, appropriate, safe and effective dental care, treatment and advice for all our patients. Fulfil any contracts we hold in relation to their care. For business administration of their care ​ During the COVID-19 pandemic, we may also collect and store personal data about our patients for the purposes of protecting public health and monitoring and managing the COVID-19 pandemic. ​ Personal data held for our patients ​ The personal data we process (processing includes obtaining the information, using it, storing it, securing it, disclosing it, and destroying it) for our patients includes: Name, address, date of birth. Unique identification number. Next of kin. Email address. Phone numbers. GP contact details. Occupation. Medical history. Dental care records. Photographs. Family group. Payment plan details. Financial information. Credit cards receipts. Correspondence. Details of any complaints received. ​ We keep an inventory of personal data we hold on our patients and this is available for patients on request. A list of personal information held is also included in our Privacy Notice that is given to all patients. ​ Disclosure to third parties ​ The information we collect, and store will not be disclosed to anyone who does not need to see it. ​ We will share our patients’ personal information with third parties when required by law or to enable us to deliver a service to them or where we have another legitimate reason for doing so. Third parties we may share patients’ personal information with may include: Regulatory authorities such as the General Dental Council or the Care Quality Commission NHS Local Authorities Dental payment plan administrators Insurance companies Loss assessors Fraud prevention agencies In the event of a possible sale of the practice at some time in the future. ​ We may also share personal information where we consider it to be in a patient’s best interest or if we have reason to believe an individual may be at risk of harm or abuse. ​ Personal privacy rights ​ Under the GDPR and the Data Protection Act 2018, all individuals who have personal information held about them have the following personal privacy rights: Right to subject access. Right to have inaccuracies deleted. Right to have information erased. Right to object to direct marketing. Right to restrict the processing of their information, including automated decision-making. Right to data portability. ​ Patients who wish to have inaccuracies deleted or to have information erased must speak to the dentist who provided or provides their care. ​ Legal basis for processing data held about patients The GDPR requires us to state the legal basis upon which we process all personal data for our patients and it requires us to inform our patients of the legal basis on which we process their personal data. This is clearly stated in our privacy notice that is given to all patients. ​ The legal bases for recording individual types of data are recorded in our patient personal data inventory. This is available for all patients to see on request. The legal basis on which we process personal information for our private patients is that it is necessary to provide the services set out in a contract − that is, to provide you and your dependants with our products and services and/or it is required or allowed by law. ​ In addition, the legal basis on which we process personal information for our payment plan patients it is in our or a third party’s legitimate interests (Privilege Plan). ​ Automated decision making ​ All individuals who have personal data held about them have a right to object to their personal data being subjected to automated decision making. Patients will always be asked to give specific, informed, verifiable, opt in consent for any processes involving automated decision making. ​ Consent Sapphire Dental will always obtain specific, opt in consent from our patients for direct marketing information. This is done via the Clinipad via an electronic contact consent form. ​ We will also obtain specific, opt in consent from our patients for certain dental treatments, photographs, and publication of testimonials. ​ For a new patient,we obtain consent for these things when the patient first attends the practice. For an existing patient, we ask the patient for consent when they attend for their recall appointment or for a treatment appointment. We refresh this consent annually when the patient completes a new medical history proforma. ​ Withdrawal of consent ​ Patients who have given their opt in consent have a right to withdraw their consent at any time. Patients are advised of their right to withdraw their consent for anything they wish to withdraw from in our privacy notice. ​ Consent during the COVID-19 pandemic ​ The rules on processing and sharing information during the COVID-19 pandemic have been relaxed to allow healthcare providers to protect public health and manage the spread of the virus. Sapphire Dental may need to contact our patients, their families or third-party organisations regarding patients’ appointments, medical histories, and treatment. Where possible, we will obtain prior consent to process and share this information. ​ Where patients have not previously consented to, or have withdrawn their consent for us to contact or share or process their data, we will assess whether the need for processing is proportionate and necessary in relation to the risk to the individual and/or the public if the information is not shared. ​ Retention period ​ This practice retains dental records and orthodontic study models while the patient is a patient of the practice and, after they cease to be a patient, for at least eleven years, or for children until age 25, whichever is the longer. ​ Complaints ​ All individuals who have personal data held about them have a right to complain. All complaints concerning personal data should be made in person or in writing to Manoj Tank. All complaints will be dealt with in line with the practice complaints policy and procedures. ​ Transferring personal data outside the EU ​ Patients’ personal data is not transferred outside the EU. ​ This Policy was reviewed and implemented on: 26/12/2020. ​ This policy and relevant procedures will be reviewed annually and are due for review on 04/12/2023 or prior to this date in accordance with new guidance or legislative changes.
